Program designed specificall to sustain independence amont seniors by providing and/or coordinating a comprehensive reange of support services all through a single network. Services include: physician service including specialists, hospitalization, nursing care, medicare rx prescription drug coverage, medication, monitoring, physical, occupational and speech therapy, personal care, recreational therapy, meals and nutritional counseling, eye, dental and foot care, social services, home care and companion visits, transportation to Senior Life center, and all medical appointments.
